  1. I didn’t understand from the picture how anyone got killed. But I read up and the crown sheet failed, which is the roof of the firebox. Wasn’t covered with water, got very hot, water sloshed onto it and pressure spiked. The firebox is at the back end. So despite the back not being blown up like the front, the crew would still have been blasted with steam. Even though that’s not the brunt of the explosion, it’s still plenty to kill
