  2. because technically they earn less than you. :) the lavish mansion they live in belongs to a llc/trust/whatever and the cars are on leasing by said company. besides their low personal income such companies are burdened with all costs, which lowers the company's taxable revenue as well.

  3.  @FuriousPope  also likely: they are no US citizens anymore, as soon they reach a personal threshold. :) USA allows taxation by citizenship, which entails not only taxation but lot of paperwork to report back each and any asset and banking account AND nasty fines if you fail to report. sometimes it is easier to renounce your citizenship and be done with that bureaucracy in case you are blessed with a clear and present line of ancestors from somewhere else to claim another citizenship. otherwise do research for CBI and turn for example into a caribbean citizen who lives everywhere but in USA.
