I grew up in a farming area and have seen this done. The poor cow is in so much excruciating pain from bloat I don't think they even notice the trocar going in. No anaesthetic needed. Where I was raised, no farmer would let a cow suffer for the time it would take for a vet to arrive. Farmers did it themselves - maybe call a neighbor in if only the wife or kids were home. The cow is typically in so much relief when the gas is released, if they don't go off to sleep like this one did, they'll sometimes turn round and lick the hand of the person letting the gas out.
