You can blame the Prison Director, Administrator and Guards but it's the overseeing body the Ministery of Justice and it's boss the Minister of Justice who should be investigated and at the minimum be put on administrative leave and if involved by negligence sacked and prosecuted, if directly involved put in prison themselves. This can't happen without that part of the Government atleast being involved, maybe it will lead all the way to the top, if that's the case this case will go no further then the Director, Prison Administrator and a few senior Guards being prosecuted and that will be the end of it !
