"An" only applies when the H is silent, e.g. "an hour", "an heir", "an honest", "an honor", and (depending on dialect) "an herb."

It's quoted out of context. Consider the situation: it happened during the 1989 Davao City hostage crisis. Prisoners rioted and took several missionaries hostage. They raped the women, including the Australian missionary in question. She later died (by a botched sniper shot), along with several other hostages and most of the rioters.
It sounded bad because Duterte was recounting the incident in Tagalog, which is not his native language. Further bastardized by being translated word-for-word to English. "Mayor should have been first" is a stilted version of what it actually meant in context. That it was massive waste since the woman was so beautiful, that not even he as the mayor had ever been (sexually) with a woman that beautiful before.
Yes it was crude and tasteless, but he wasn't saying that he should have "raped her first" as so many international news articles later spun it off as.
And yes, he's obviously not the shining bastion of political correctness. LOL. But even so, he is still far more liberal in terms of policies than virtually all of his peers. That irreverence for the status quo is actually what makes him able to do that, in a country where conservative Christians have historically vetted most politicians and still strongly influence legislature.
He does openly admit to extrajudicial killings, which is horrible in its own way. But again, context. This is the Philippines we're talking about. It's barbaric in terms of human rights, but it may actually be the only remaining solution in a country where crime and corruption have become so rampant. It's either that or become another Mexico.
But that's not the point.
The point is that he is nothing like Trump.
