I remember reading a book called 'Once a Warrior King' by a special forces guy serving in Vietnam and he explained how they would have to fill out endless reports on how things were going in their area of command. If they wrote anything negative they were often fired and replaced, rather than consulted on how to change the situation. So Westmoreland and his clique of geniuses were getting all these wonderful reports on how fantastic everything was until the Tet Offensive proved otherwise.
