I worked in health care between 1996 to 2007 and staffing was always a issue. A facility I worked did try to improve this, but found that there was a shortage in skills. People not applying! How in that particular care setting was high dependency long term end of life, we made the residence relatives aware on the staffing issue and what that means. All our focus was on residence care, means other domestic task was put back. Just the appearance of the unit, but none of the residence got missed. It means the residence themselves wasn't rushed, they got the time needed. But it needed the quality in the degree... qualified trained staff.

I worked in health care for over ten years and don't regret the time, as I achieved. I can sympathise with many concerns raised here and it can be shattering for the families. I agree that long term care in an institution isn't necessarily idealistic for independence and adaptability. However the enormity within a person care maybe not properly assessed (if that care was in their home). Dementia.. when managing aggression, combination from medication and risk assessment together. (As in care plans). I can't empathize harder the importance in documenting and referral when confronted with those challenges. Long term care shouldn't be isolating. A lot is about staff training. You can blame staff so far, but management got to be stronger.
