  2. Collapse of a completed building is rare anywhere, and unheard of in the USA, but the US state I would have voted most likely to have such a disaster happen is Florida. That is where the infamous FIU pedestrian bridge collapsed when the structural elements had all been placed.

  3. This should not happen anywhere. Any place that has the money and technology to build such a large structure has access to all the engineering expertise and technology needed to build a safe structure.
