  1. My completely off the wall guess would be improper grounding of the second stage fuel tank and fueling interface which led to a static charge buildup and discharge, igniting the fuel and leading to the rapid unintended chaotic disassembly of the vehicle.  Though I am biased toward this hypothesis after witnessing a JP8 tanker to tanker transfer gone wrong (ie explosion) for this reason.
