DSM has very grave objections to it. It is not used outside the USA. It is dangerous to regard it as an authority.

Swyer's syndrome - complete insensitivity to androgens including testosterone - is extremely rare. These people (XY) look female from the outside and think of themselves as female. But they do not have a female genital tract - ovaries, Fallopian tubes, uterus, cervix, full vagina. For that to develop, the fetus must have sex chromosomal constitution XX, with no Y chromosomes. The person with Swyer's syndrome usually has it diagnosed when menstruation - which is expected - does not occur. Since the person thinks of themselves as female - their brain will be totally insensitive to testosterone - they are normally assisted in a female direction by having a full vagina constructed so that they can have sexual intercourse. They often marry men and adopt children.
