  1. Charing Cross Hospital in London's Accident & Emergency Department failed to diagnose my broken arm - I had tripped and fallen on the pavement. Apparently, they could not read X-rays. I thought it felt broken, but having been told adamantly that it was not, I inflicted further damage on it. It was finally correctly diagnosed by a consultant orthopaedic surgeon six months later. I have had 18 months of pain and inconvenience.
