  1. I think the problem was distinguishing a live round from a non-firable dummy round. Dummy rounds contain no powder, but they put a BB in the empty shell case to help identify them (you shake the round and can hear the BB rattle). Perhaps the 1st AD shook the entire pistol, heard the BBs rattle in two of the rounds and assumed the third round was also a dummy.
