If there isn't any sort of record of the owner requesting that change, there's probably some hope of suing the insurance company.

With the amount of money on the line, it's probably worth taking a close look at how that policy change came to be made. It might be that he did ask for it to happen, but didn't realize that there was a change made to that or it might be that there was a change made and that he initialed the paper work without realizing that the coverage had been dropped. But, also keep in mind that the burden of proof is on the plaintiff and so just a lack of specific instructions to remove the coverage might not be enough.

The damage to the bus would be covered by that, the damage to the car would not. Presumably, because the cleanup of the scene, and likely towing, are of the car's parts more than the bus, it would be covered under any policy covering the cost of repair or replacement of the car. But, IANAL and I don't really have access to any more details than this.
