Re: USB intermittent operation - I have had this issue corrected on some systems with a MB firmware update and Windows Vista being a shite. Otherwise boot linux and run some monitoring software to see if the USB drops out over a few hours of automated testing, if you don't get any issues on linux running then you know it's probably software not hardware, if you get issues then you end up in the sit where it could be any number of issues.
