  1. Electric cable colors are always Green White Black and then Red. They don't make special cables for 240 volts with a red and no white for no neutral. So the white then becomes hot on 240 circuit. That looks like you installed an outlet with undersize gauge wire. Should be using #8 copper if you want to use a full 40 amps breaker in the panel.
