I work in this industry. I ran the factory floor systems for the C-17 cargo jet. It’s almost impossible to leave parts off of the airplane. I am assuming that they left off 4 bolts here. The mechanic would see the 4 bolts in his job box. The mechanic would have to buy off the job in the system. I assume that there were inspection steps and that an inspector would have to look at the finished assembly and then buy off the job. So, multiple fails if this is what happened. I have no insider information. I fail to see what additional laws could help. This seems to be attention to detail. Just follow the current procedures, find out how these escapes can happen and plug the holes.

@billj4137 I am only guessing. However, I saw the design. It’s 4 bolts with castellated nuts and cotter pins. If they only hand tightened the nuts, the mechanic would still have 4 cotter pins in the box. If the nuts were installed but not torqued and then the pins were put on. No problem. It would rattle but never come off. The bolts only keep the door from moving up which is how it would unseal. The door is installed on two post/hinges. It slides down on those posts and engages the door latch posts which seal the door in the same position as the doors that normally open. In other words, the bolts are not under load to retain the door (much). They merely keep it from moving up and out. The latch posts on the doorframe and the hinged posts at the bottom of the door are the primary structural parts. Time will tell what the root problem is, but I’ll guarantee it’s something wrong in the assembly. And it was delivered to Boeing that way.

@billj4137 Who knows? I am just guessing. But do have industry knowledge not specific to this case.
