  1. I live in the USA. 2 years ago, I had a colonoscopy. They sent paperwork with appt. card a few weeks after at 11pm. 3 phone calls to get them to clarify if it was 11am. They were nasty, said not enough staff to answer their calls. I went to the appt. It took <30 seconds for the assistant to tell me everything was fine. Copay: $75!!! A week ago, I went to my routine knee surgeon appt. after I had surgery last year. They refused to see me. I pay my insurance every month (~$165 Obamacare, gov't pays ~$435!). I would have paid the copay of $65. But I did not have a new referral after I changed insurance. Horrible.
