  1. I don't think any of us will be in the least surprised by this criminally low rate of reporting. As is only too clear a massive effort is being put in to sweep information about adverse reactions under the carpet. The most troubling aspect is how keen so much main stream media is to lend a hand in this. Whatever happened to Google's moto: "don't be evil"?
