@John-gm5mf No it was an accident. The actor firing the weapons was not to blame. The master of arms was to blame for not properly checking the firearm. I believe a new code of conduct was introduced.

@John-gm5mf He was killed on the set of the Crow. A dummy bullet had fallen into the barrel of a prop-gun. So when blanks were fired at him the bullet came out of the barrel and killed him.

@betsybabf748 That was the guy in the TV show Cover UP. He was playing Russian Roulette with a blank firing magnum. He lost and the shockwave caused by the blast cracked his skull like an egg.
