Sounds to me like the issue here was that at least one of the two captains, if not both, was a narcissist which explains how they didn't want to correct their course to avoid a collision. Narcissists are well-known for not admitting they're wrong or caving in when they think someone else is wrong (and course correcting in this case would be seen as an admission they messed up), and being impatient to the point they will break whatever rules they have to to get someplace/get something done faster, even if it means being unsafe. This is also what happened in the Tenerife air disaster of 1977, which still remains the deadliest air disaster in history. So, basically, narcissism has caused two of the biggest manmade disasters in recorded history, more or less. Anyone that's had the misfortune of living with/ knowing a narcissist can tell you they're one of the most insufferable types of humans. And, as we see here, potentially one of the most dangerous types.
