No one, including this overeducated but completely unwise physician, has got the balls to state which is clear to anyone with any sense of logic: Those that have mental health issues get treated whether they want to or not. Sorry about your freedoms and privileges. They'll be revoked until you either get sober or the mental schism is resolved. Those that are experiencing temporary homelessness(people that actually want to work and have lost their residence due to economic upheaval or a single traumatic event but are otherwise mentally fit and well) will be given the help they need to get back on their feet, both medically and economically. Those that refuse to be "homed" or "helped medically" will be given a space where they will be forced to fend for themselves...forever. No handouts, no drugs, no medical assistance. No need of society? Make your own and best of luck.

The United States, like other FREEDOM loving states, decided that even the mentally ill had the RIGHT to live like animals, defecate in their pants, abuse drugs and not to be held to account, because placing them in care was fundamentally against their RIGHTS and FREEDOM. This is why Portland is an open toilet, a giant festering potty, not portable but static and it will remain so until someone who thinks logically and rationally decides to say: You're being treated whether you like it or not. See how that works?
