  1. I'm not aware any airborne operations took place over anything but small towns. However, Operation Market Garden showed that church steeples of even small towns could be a hazard.

  3. Not like NYC. It's the location of Newark Airport compared to Manhattan that has resulted in a number plane crashes. The normal approach at one time was within a few miles of Manhattan. That was changed after the 1946 crash to take incoming flights furter from Manhattan, but flying in instrument conditions with no instruments means you still need to get low enough to see the ground before landing. All it takes is being a few miles off in a high hazard location like Manhattan.
