  3. Well in pure theory it is possible to measure productivity. You just need a company with only 1 developer. You check how much money the company makes with its product and how much it costs and you have productivity. Problem is that this scenario is not very useful.

  4. I think you underestimate things. This happens exactly the same in a hospital. The only difference is that people are very very careful at saying life A is worth more than B.
