  1. It's selfish drivers. They enter the freeway to slow, causing someone to change lanes, which slows another driver, who changes lanes. Next thing you know, it's all slowed down.

  3. I was pleasantly surprised to find an express lane in one midwest city (Hwy 70, I think). Limit the points where you can enter or exit the express lanes and physically separate them from other lanes so local traffic cannot benefit from clogging them.
