Yes there was a failure on the part of the school staff, they let the parents refuse to remove him from the school! Beyond that it's the parents that allowed this to happen.

@iamlrrrruleroftheplanetomi1799 No his parents were given the choice to remove him from school and they refused. Yeah they shouldn't have been given a choice but hey they are his PARENTS.
