That rule is exploitable, leading to weaponized incompetence. Malicious parties will design systems to appear incompetently designed specifically to mess with you. That the same system could have been produced through sheer incompetence provides plausible deniability. While PDF forms are unreliable, when the fields are there it's obvious they should be inspected, and it's dishonest to slap a person's name on the response if the person wasn't seeing the submitted material.

Fun followup assuming this is true: How many other parties have been sent his particular "blank" form, with all those personal details?

 @McTroyd I don't think it's specific to him. I expect the repair program's overarching strategy is minimal service for maximum inconvenience. They don't need this data collection, they don't need to sabotage lid sensors. They merely choose to make it hard.

Why assume an absence of malice just because there's incompetence involved? This process is difficult and its feedback unhelpful by design.
