Its a common misconception that most people died at that early age, even a slight uptick in infant and childhood mortality can have a massive impact on the average life expectancy. If you have a population of two, and one dies at birth and the other at 90, the average life expectancy is 45 years. Very quickly you can see the downward average this can have. Its a fallacy that modern medicine has extended life much. What it has done, is massively reduced infant, childhood and birthing deaths. Reduction in the amount of wars and the amount of men who die in them also help raise the average life expectancy. Civic engineering, which the Romans started with their massive sewer systems, also helped in the 19th century.
