It can happen earlier too. It probably isn't rage, but frustration. Several years ago I had a neighbor who would occasionally get confused easily. He'd walk to the store a couple blocks away and forget his way home. Likely confused because he thought he lived at his old place and didn't know why he was in the new neighborhood. Or knew he moved, but didn't recognize any of the landmarks. When people tried to help he'd start yelling at them. Again, frustrated because strangers were asking him seemingly insane questions. Most of the time he was perfectly normal. But every few weeks he'd be back to Angry Old Man mode. Sadly, he didn't remember these episodes after they happened and probably didn't understand why some people avoided him. President Biden was probably at this stage when he was elected. That's why he publicly complained so rarely. His wife and staff knew he could handle a scripted event. Being a politician for decades it was instinct to him. But when challenged with something off script he'd get frustrated and strike out. Look at some of his angry responses when asked questions he didn't have a rehearsed answer to. His staff could see this coming. That's why they regularly "guided" (Forcefully pulled) him out of these situations. A Presidential candidate who tries to throw down with a random voter for asking an uncomfortable, but reasonable question doesn't make for good optics.

I'm convinced that Jill Biden, VP Harris, and a small group of others have looked at Pres Wilson's final years after he had a stroke. His wife became the defacto President and just signed his name for him. Today this would be harder to do. But with the right people to help (Like a Vice President and cabinet members), they think they can prop Joe up in the corner and run things themselves. As long as he has a pulse they keep power. If he drops dead after November, they still get to keep the power.
