Every single unprovoked fatal dog mauling I've seen involves dogs not documented to have been abused. In fact, large minority cases, the dogs have been documented to have been spoilt rotten by the owner from puppyhood for years, till the doggie decided to play alpha, and bit the owners for not behaving like beta.
