The emptiness here is the sufferer's feeling of unfulfillment, of not becoming what they feel they were meant to, of a worry that existence itself is empty. It's not that there's no substance to the person, which is what it sounds like you meant. I refer to the general case, and don't mean there can be no exceptions.

Let's not forget, btw, that therapists and psychiatrists frequently suffer from maladies and profound damage themselves, and treat their patients cavalierly, summarily and even with hostility. They also commit grave errors in professional judgment, often due to an opinion of themselves which is too generous. Some just aren't clever enough. This is more a reaction to the remarks down below than to the video itself.
