  3. Have a look at the Mooienhof-Kuipersdijk intersection in Enschede. There is a bicycle lane squished between a car lane and a bus lane without any dividers; it's painted on. Granted, it's not likely to be used all that often as most users won't go straight across the intersection, but the fact that you can design it that way and see nothing wrong with it is amazing.

  4. Usually it's done through spot checks. Also, lowering the cost of employees can be balanced against the occasional theft. The customers are either sufficiently civil people and/or an awareness that getting caught is a likelihood above a certain threshold.
