The 9×19mm Parabellum (also known as 9mm Luger, 9mm NATO or simply 9mm) was originally designed by Austrian firearm designer Georg Luger in 1901. So basically we're talking about a cartridge that's well over 100 years old. Yes, there have been great improvements in 9mm cartridge design since then, but it's still about as old as the .38 Special cartridge. Whereas the .40 S&W (10.2×22mm) was developed jointly by American firearms manufacturers Smith & Wesson and Winchester in 1990. It was engineered from its inception to be a modern, high pressure (35,000 psi), high performance (468 ft⋅lb) self-defense cartridge. Just because the 9mm is currently in vogue doesn't negate the superior ballistic performance of the .40 S&W.

Best barrel length for non-military and non-LE use is the 18" CMV with mid-length gas system. Gives almost the same FPS velocity as the 20" barrel, while being almost as easy to handle as the 16". Now if you're military or LE that needs to go in and clear a building, then that's another story.

@WilliamRepsher Inside vs. outside. If I'm in my own home and need to protect myself and my family from an intruder breaking in, then a handgun (or maybe a short-barreled shotgun) would be my weapon of choice. However, if I'm faced with an external threat (e.g., an angry mob) and need to protect my property from said threat, then an AR-style rifle would be preferred. As for most starting points for a gunfight being inside, couldn't really say. One needs to be prepared to face any situation that poses a threat to life or property. Cheers.
