It can mean both. Like 95 percent efficacy means you've got a 95 percent lower chance of getting it at all. Then the chance of it being serious are a separate metric. It's like chickenpox. If you've had it once, you almost certainly won't get it again, but if you do, it almost certainly won't be serious
