  1. Nearly a factor of ten more costly to operate than 'expected' is certainly a major fail in THAT respect. NASA is inherently grossly inefficient at building any large system in part due to perverse necessity to have some nut or bolt built in every congressional district in the nation. They need to stick to building exploration probes and such not build STS or any launch systems now that privateers have proven to be 5 to 10X more cost effective.
