  1. More to the point, the failure of those involved in design, testing, procurement, etc. If it happened then, is it happening now . . . ? (Space Shuttle 'o' rings anybody?)

  3.  @historigraph  I have read that the US forces used the term 'Ultra' when describing their own codebreaking. I can't be specific, sorry. Bear in mind that US ground and air forces, less so navy, were in the ETO and supplied there with info from Bletchley Park. They might have found it difficult, subsequently, to differentiate 'Ultra' from 'Magic' and, as end users of restricted info, why would they care?
