  1. The reward structure around these projects is what dooms them from the start. Starting with politicians who are rewarded for looking decisive and strong by kicking off mammoth projects, weak and ineffective by taking a smaller, incremental approach. As long as politicians are rewarded for looking busy these problems will persist, and some software contractors will continue to make billions in profits for not doing anything useful.
