I think his mental illness stems from realizing how out of touch with reality affluent people like himself are. Rich people have built a bubble around themselves and now the bubble is bursting perhaps. Trump has never been grocery shopping since his servants do the shopping for him. He was shocked to learn you don't need photo ID to buy groceries. Having to change your paradigm at his age makes you crazy since you're so old and set in your ways. Also the pain of being president and realizing that politics is more different than business than you assumed it was causes mental anguish. Perhaps Trump's whole worldview is crumbling and it since he's aged and set in his ways this makes him crazy.
