@mattsocal2010 India and Russia have similar systems, so the argument of population ot size simply doesn't work. Even places with "rugged individualism" (like Switzerland) have better outcomes for 99% of people. Of course if you need cutting-edge technology then you can go to America but most people cant afford that anyway. So it's better to have a "VA" system than what you have now. Do you complain about electricity or the mail? If something works well, you don't even notice it.
