Magneto (X-Universe) : formerly trained by Professor Sebastian Schmitt (Sebastian Shaw's past identity) and NZis. Later on partially trained by the C...A-funded first iteration of the X-Men, the Hellfire Club (whom he became the leader after eliminating Shaw) and later on founded the Brotherhood of Mutants, then an underground army of Mutants (and/or allied with the Four Horsemen of Apocalypse, depending the timeline) . The C...A has never been blamed for these neither.
The C...A A$$assin who was totally not Frank Castle (TASM2 / Webbverse) : sent by Oscorp and the C...A to eliminate Peter Parker's parents-- who were implied to be presumably undercover C...A agents too. Once again, the C...A wss never blamed for that neither.
C...A (Blade movies universe) : every single body of authority, government and intelligence agency has ties OR are under the Vampires's steed. Everytime that Blade and his mentor get caught up during the trilogy, it was partially because some vampires and their human allies were gathering intelligence on the vampire hunterd, weakmesses, strenghts and whereabouts. The archnemesis of the Blade Trilogy has never been Dracula / Drake nor the Vampire shadow governments: it was wyte suprematist paranoia toward a superpowered black man gallivanding around free, and the human esthablishment all along!
And don't let me start with the C...A in DC movies. The Dark Knight Rises (Nolanverse) ?? It all started because a C...A intel agent scr*wed up and get caught by Bane. Batman V Superman (DC Extended Universe) ??? It's all Jimmy Olsen (an undercover C...A agent) 's fault!!! S...ide Squad 2016??? A C...A-funded mercenary squadron who played with fire by letting a 6,000 years old transdimensional Homo Magi suprematist sorceress going around free!
