Standard procedure SHOULD be to give a fresh vaccine... The obvious question is if they use the same injection a second time, when blood has coloured the vaccine, how will they know if they hit a vein on their second attempt? ...People are lazy, who knew?

We now know that some batches were placebo. My suspicion is that when they saw the high level of adverse reactions they started diluting the batches with placebos to conceal the level.
