No instruction/safety manual + inadequate training = disaster. The US has a history of limited regulation, infrequent inspection, poor enforcement & toothless watchdogs. Very little is done even after accidents & the fines are pitifully small.

This is an example of Amateur Hour from start to finish. That no one was held criminally liable was an abject failure to take health & safety seriously. No fire risk assessment, no emergency plan, no emergency exits, having doors & brakes on the same system meant no redundancy, no emergency lighting, plastic piping ( save money) instead of metal. When you prioritise vandalism over safety that is negligence. This was criminality not "honest mistakes".

I can't remember now where I got the info, whether it was TV or radio. The space shuttle was a flawed design because it was made much larger than originally planned, specifically the cargo bay was made bigger due to the CIA wanting it to be ably to put in orbit large spy satellites. By increasing the size of the shuttle more problems were introduced because of the weight of the vehicle to get it into orbit and needing to protect a large area upon re-entry. Then because of manufacturing costs there was a need to re-use shuttles more than intended by the original plan.

@jerwatson79 This has already be debunked! It was the design plus the heat from the jet fuel producing molten aluminium from the aircraft. Once 2 or 3 floors collapsed it became a domino effect.

One very important point missed out is that the Horizon system software was using Visual Basic a variant of BASIC. This is NOT commercial software! It was developed by Microsoft for home users to learn programming. So for Fujitsu to use it for this scale of project was down to greed as it didn't have anywhere close to the capabilities of say Excel ( you can do calculations in a Word document but you would use it for business accounting) A relative worked in the Civil Service in procurement: he got fed up with Fujitsu going over budget with long delays so he took them off the tender list for new contracts. Long story short a minister insisted the be put back on the list. Of course they won the next contract & surprise, surprise the project had significant delay & went well over budget.

Not sure what the firefighters were hoping to achieve by spraying water onto a mysterious cloud. Another case o doing too little, too late.
